Please fill out the form completely, and if submitted by the deadline, your comments will be read into the record during the appropriate time at the meeting.

  3. If you would prefer to speak live during the meeting, please check the box below and the meeting information will be provided to you via email. This request is required to be submitted no later than twenty four (24) hours before the start of the meeting you wish to speak during.
